Estimation of the post-mortem interval using microRNA in the heart in aluminum phosphide deaths

Master Abstract

SUMMARY post-mortem interval (PMI) which is the duration of time that passed between the precise moment of death and the postmortem examination is considered a focus of inquiry in the forensic discipline due to the potential for both civil and criminal consequences. In order to establish a more precise calculation of PMI, forensic pathologists have been able to carefully assess the rate of biological markers' degradation (such as proteins, DNA, and RNA). Following an organism's demise, the RNA degradation and the loss of particular RNA transcripts seem to be very susceptible in terms of speed and temporal correlation. According to the kind of RNA, post-mortem stability varies. Particularly, miRNA is more stable than longer RNAs like mRNA. One of the agrochemical pesticides used to boost agricultural productivity is Alp. Because it is readily available at a low cost, it is frequently used as suicide poison. Alp is becoming a common self-poisoning agent in Egypt. Cardiovascular issues accounted for 70% of Alp-related deaths. The aim of the current study is to test the possibility of estimating the PMI using miRNA-21 and HIF expression levels in the heart samples from Alpt. The current study was a cross sectional study on 60 post-mortem samples (heart tissues) collected at different intervals during forensic autopsies. The two groups were allocated equally according to the cause of death into Group I (control, n?30): Deaths caused by other than toxicity, and Group II (cases, n?30): Deaths due to Alpt. MDA and GSH, were measured in heart tissues using ELIZA. miRNA- 21and HIF expression levels were measured in heart tissues at different PMI using RT-Q PCR. ROC curve for detection of cases using miRNA and HIF was carried out. In the current research 51.7% of aluminum phosphide deaths were male, 82.8% were from rural areas with mean age 27.83 ± 15.44 while 48.3% of no toxication control deaths were female with mean age 29.41 ± 11.98 and 51.7% from rural areas. The commonest causes of death in no toxication control deaths were hemorrhagic shock (51.7%) followed by brain trauma (27.6%) and only (20%) were mechanical asphyxia. The mean PMI in no toxication control deaths was 29.19 ± 19.63 while that of aluminum phosphide deaths was 38.95 ± 31.88. miRNA-21 expression levels in aluminum phosphide deaths were up regulated when compared to no toxication control deaths with mean expression (1.17 ± 0.68, 0.87 ± 0.68 respectively). HIF-1a expression levels were up regulated when compared to no toxication control deaths with mean expression (84.49 ± 7.69, 20.24 ± 7.37 respectively). GSH was statistically significant difference between aluminum phosphide deaths and no toxication control deaths with mean value (1.28 ± 0.51, 0.98 ± 0.40 respectively) while the mean MDA of no toxication control deaths was 2.14 ± 0.83 while that of aluminum phosphide deaths was 2.27 ± 0.68 with no statistical significance. There was negative correlation between GSH and MDA in both groups. There was positive correlation between miRNA-21, HIF and MDA with PMI while there was negative correlation between GSH and PMI in both groups. In prediction of post mortem interval in aluminum phosphide deaths miRNA-21 sensitivity was 75.9% and specificity was 51.7% while HIF sensitivity was 100% and specificity was 100%. There was positive correlation between miRNA-21 with MDA and negative correlation with GSH in both groups. Our study showed that there was positive correlation between MDA, miRNA-21 with HIF while there was a negative correlation between HIF with GSH in both groups. The results of this study indicate that the PMI can be calculated using the degree to which particular miRNA-21 and HIF are expressed in heart tissue. The combination of miRNA-21 with HIF in post mortem estimation are precious indicators and being a useful tool in postmortem diagnosis of aluminum phosphide-induced myocardial injury.
